All materials are due at Manual by January 16, 2009 unless otherwise noted.  Please make a copy of submitted materials, since they will not be returned.  Enrollment is limited.  Submission of an application does not guarantee placement in a program or school.

MST Application Code: 200/MGD

1.	A completed application form designating the desired magnet program (delivered to the office of Optional, Magnet, and Advance Programs by January 9, 2009).
2.	A writing portfolio quality essay, composed and written by the applicant, in response to the following question (delivered to Manual by January 16, 2009 ). The Twentieth Century brought an explosion of discoveries and inventions in math, science, and technology. Choose one such discovery or invention, identify it, and name the people responsible. Then explain how this discovery or invention has influenced your life. Limit your response to one typed page.
3.	A completed activities and interest survey* form which describes the applicant's activities, volunteer work, awards, and recognitions--especially those that relate to the desired magnet program (delivered to Manual by January 16, 2009 ).
4.	Two completed recommendation forms* from teachers (one must be the current math or science teacher), a counselor, a principal, or any adult familiar with the applicant's abilities, work and commitment (delivered to Manual by January 16, 2009 ).
5.	A copy of the applicant's school transcript which should include attendance data, achievement test scores (CAT-5, CTBS, Stanford), advance program test scores, CATS scores, past grades, and current progress report (delivered to Manual by January 16, 2009 ). -- Be sure to include your most current progress report card.
